AMPK Phosphorylation

AICAR is taken up by cells and converted to ZMP, a structural AMP analogue. ZMP activates AMP-activated protein kinase (AMPK) by allosteric activation and inhibition of its dephosphorylation — the same cellular energy sensor activated by exercise, caloric restriction, and metformin.

PGC-1alpha and Mitochondrial Biogenesis

AMPK activation phosphorylates PGC-1alpha, the master regulator of mitochondrial biogenesis, increasing the number of mitochondria per cell and upregulating oxidative enzymes. This is the molecular basis for the endurance and aerobic capacity improvements observed in AICAR-treated subjects.

Fat Oxidation and Substrate Switching

AMPK activation inhibits acetyl-CoA carboxylase, reducing malonyl-CoA levels and de-inhibiting CPT-1 — the transport protein that moves fatty acids into mitochondria for beta-oxidation. The net effect is a shift toward fat as the primary fuel substrate.

In practice, for human users who do exercise, it amplifies the AMPK signal beyond what training alone achieves — more mitochondrial adaptation per unit of training stimulus.
AICAR is banned by WADA in competitive sports due to its endurance-enhancing mechanism. Users in competitive sport should verify their governing body's substance policies. For non-competitive research and longevity use, it is a legal research compound in most jurisdictions.
Both activate AMPK but through different mechanisms. Metformin inhibits Complex I of the mitochondrial electron transport chain, indirectly raising AMP:ATP ratio. AICAR directly provides a ZMP that allosterically activates AMPK more directly. AICAR is generally well tolerated in short-term research. The primary reported concern at high doses is lactic acidosis in cell models (AICAR can affect mitochondrial respiration at very high concentrations), but this has not been clinically observed at human research doses. Monitoring lactate during early use at higher doses is a reasonable precaution.
