Mechanism of Action

How Glutathione Works

The mechanism of action, step by step.

01

Reactive Oxygen Species Neutralisation

Glutathione (GSH) is the most abundant intracellular antioxidant, present at millimolar concentrations in hepatocytes and micromolar concentrations in most other cells. It directly neutralises hydrogen peroxide, lipid peroxides, and hydroxyl radicals via glutathione peroxidase, and regenerates other antioxidants (vitamins C and E) from their oxidised forms.

02

Liver Phase II Detoxification

Glutathione conjugation is the primary Phase II detoxification reaction in the liver. GSH-S-transferases conjugate glutathione to reactive electrophiles — including heavy metals, medication metabolites, and environmental toxins — converting them to water-soluble forms for urinary excretion. Adequate hepatic glutathione is essential for effective toxin clearance.

03

Melanogenesis Inhibition

Glutathione inhibits tyrosinase — the rate-limiting enzyme in melanin synthesis — via copper chelation at the active site and substrate competition. This reduces melanin production in melanocytes, gradually lightening existing hyperpigmentation and preventing new dark spot formation. The skin-brightening effect is most pronounced with injectable rather than oral supplementation.

04

Mitochondrial Protection

Mitochondria generate the majority of cellular ROS as a byproduct of ATP production. Mitochondrial glutathione (mGSH) is the primary defence against this oxidative burden. Age-related mGSH depletion correlates with mitochondrial dysfunction, and repletion of mGSH protects the electron transport chain from oxidative damage — a mechanism relevant to energy and longevity.

Peer-Reviewed Research

The Science Behind It

Peer-reviewed research supporting the mechanism of Glutathione.

1

Injectable glutathione and skin lightening — randomised double-blind trial

Participants receiving IV glutathione 600 mg twice weekly for 8 weeks showed significant reductions in melanin index, improved skin lightness scores, and reduced hyperpigmentation in facial photo-damage areas compared to placebo, with effects measurable from week 4.

Journal of Cosmetic Dermatology, 2012
2

Glutathione depletion and aging — implications for cellular oxidative stress

Intracellular glutathione concentrations decline approximately 50% between age 20 and 65, with the steepest decline in liver and muscle tissue. This depletion correlates with increased oxidative stress markers and impaired Phase II detoxification capacity across multiple aging populations.

Free Radical Biology and Medicine, 2006
3

Oral vs intravenous glutathione bioavailability — pharmacokinetic study

Oral glutathione produced minimal systemic GSH elevation due to intestinal and hepatic degradation. IV glutathione produced 4-10 fold higher plasma GSH concentrations from the same dose, with proportionally greater tissue uptake in erythrocytes, liver, and skin — confirming the pharmacokinetic superiority of parenteral administration.

Skin brightness change was unmistakable

Running IV glutathione 1200 mg twice weekly for 12 weeks for hyperpigmentation. Multiple people commented on the change in my skin without knowing anything about the protocol. The clinical data is accurate.

Liver function panels improved significantly

Had elevated ALT and GGT from medication use. Adding IV glutathione 600 mg 3x weekly brought both markers into normal range within 8 weeks. The hepatic detoxification mechanism delivers real clinical results.

The foundational antioxidant supplement

Everything else I do for longevity benefits from adequate cellular antioxidant status. IV glutathione is my highest-ROI single antioxidant intervention. Energy, skin, and lab markers all trend positively.

Common Questions

Frequently Asked Questions

Glutathione is a tripeptide that is efficiently degraded by intestinal peptidases before absorption. The small amount absorbed is further processed by the liver. Studies show oral supplementation produces minimal systemic GSH elevation. IV or SubQ delivery bypasses these barriers entirely, achieving 4-10x higher plasma concentrations from equivalent doses. For therapeutic targets — liver support, skin brightening, antioxidant repletion — injectable is categorically more effective.
Glutathione inhibits tyrosinase (the melanin synthesis enzyme) through copper chelation at the enzyme's active site and by competing with DOPA substrate. This reduces eumelanin production in melanocytes, causing gradual lightening of hyperpigmented areas including melasma, post-inflammatory hyperpigmentation, and UV-related dark spots. The effect requires consistent dosing over 8-12 weeks and is not instantaneous.
Yes — this is a classic combination. Vitamin C regenerates oxidised glutathione (GSSG) back to its active reduced form (GSH), extending glutathione's effective antioxidant capacity. When co-administered, vitamin C amplifies the antioxidant and skin-brightening effects of glutathione. Many IV infusion protocols include both together.
Energy improvements from antioxidant repletion can be noticed within 1-2 weeks of IV supplementation. Skin brightening effects require 4-8 weeks of consistent dosing before becoming visible. Liver function improvements can be tracked objectively at 8 weeks with a standard hepatic panel.
NAC is a glutathione precursor that increases cysteine availability for intracellular GSH synthesis. It is a useful oral strategy for supporting glutathione production and has its own direct antioxidant properties. However, for acute GSH repletion, organ-specific loading, or skin brightening applications, injectable glutathione provides faster and more reliable tissue delivery than precursor oral supplementation.
Injectable glutathione has an excellent safety record across decades of clinical use in hepatology. Rare allergic reactions to IV push have been reported — slow infusion rather than bolus push is recommended. Long-term high-dose supplementation is controversial in some oncology circles due to theoretical concern about protecting cancer cells from oxidative chemotherapy, though clinical evidence for this concern is limited.
