Senescent cells survive by sequestering p53 in the cytoplasm via interaction with FOXO4, preventing p53 from entering the nucleus and triggering apoptosis. FOXO4-DRI competes with endogenous FOXO4 for p53 binding, freeing p53 to initiate programmed cell death.

Because only senescent cells rely on the FOXO4-p53 survival complex, FOXO4-DRI's mechanism is cell-type selective — healthy, proliferating cells express different survival signals and are unaffected by the peptide.
